[Rx2021] Flu Shots - Due by October 1, 2020

Hello Students,


Influenza vaccines are now beginning to be available at various locations. As always, please watch for postings of flu shot clinics that will be held on-campus in the upcoming weeks, as well.


Every student, P1 through P4, must receive a flu shot by October 1, 2020, so that they can participate in experiential learning activities over the next 6 months.  Especially in this time of COVID-19, flu shots will be a critical public health measure for all of us.


Please take the attached form with you when you get your flu shot and have the provider sign that you received the vaccine.  A copy of the form is also available in your CastleBranch account, and on the EValue home page. The form must then be uploaded into your CastleBranch account by October 1st.  If you have trouble uploading into Castle Branch, please let Ms. Schmotzer know and she will open up your slot for the vaccine.


Students who do not receive a flu shot will not be permitted to attend experiential learning rotations or activities.  This could impact your ability to progress in the PharmD program and/or graduate on time.
