[Rx2021] APPE Block 9 Reminders

Hello P4 Students!

You're in the home stretch!!

As we approach the end of block 9 and look forward to graduation, please be aware that all assignments, PITT form reports, portfolio work, and E*Value evaluations (by students and preceptors) need to be completed by end of day Monday, April 26th so that we can certify that you have met all requirements for completion of the PharmD program and thus can graduate on-time.

Please work with your preceptor to have them complete the Final Preceptor Evaluation of the Student for you on time.  We have a shorter turnaround time than we had with your other rotations, so please plan accordingly.
