[Rx2021] Residency Opportunity at University Hospitals, Cleveland

Dear Students,

I have received the following information about a PGY1 Community Pharmacy residency program at University Hospitals in Cleveland, OH:


We are going through the match, it is listed under UH Meds.  Our site visit for accreditation is in February.



   We are developing marketing materials, but I think I can paint the picture.  Our goal is to own or collaborate on every moment of care outside the acute clinical services inside the hospital.  If you look at the slide below it helps tell our vision;


1).  Admission med rec is a nationwide crisis.  Pharmacy needs to own that, and it is a component of our residency in July 2021

2).  Meds to Beds and Med education.  But not just that; coordination of care at discharge, and TOC of the most challenging patients

3).  Post-acute pharmacist visits in primary care.  This is where we start utilizing our consult agreement.  This is a HUGE part of what we do.

4).  Meds, we want all the meds in house.  Knowing their compliance is critical

5).  Pharmacy assistance.  We give back to the community with millions in charity care

6).  Population Health.  We have a massive telemedicine program that currently provides care to 50,000 lives.  It will be 125,000 by next July.  Another huge component

7).  Chronic Care Management. We are just developing this now, but hope to be fully competent by July.

8).  Specialty pharmacy.  Only 5-10 shifts at specialty, but they get to learn that environment, and understand the processes.  This is our only rotation, everything else is longitudinal.



We still have a couple other balls in the air.  If those come true there could be up to 23 spots.  They are with endocrinology, anticoag, and our employee wellness plan.
