[Rx2021] Flu Vaccine Requirement

Dear Students,
Influenza vaccines are now available at various locations.  Every student, P1 through P4, must receive a flu shot by October 1st so that they can participate in experiential learning activities over the next 6 months.  Please take the attached form with you when you get your flu shot and have the provider sign that you received the vaccine.  A copy of the form is also available on the EValue home page.  The form must then be uploaded into your Castle Branch account by October 1st.  If you have trouble uploading into Castle Branch, please see Ms. Schmotzer.  Students who do not receive a flu shot will not be permitted to attend experiential learning rotations or activities.  This could impact your ability to progress in the PharmD program and/or graduate on time.
