Please remember to log in to EValue and complete your IPPE Student Final Self-Assessment and your Evaluation of Preceptor and Site.  Also, please remember to turn in to Ms. Schmotzer in Student Services your completed Hours Documentation Form (this can be found in Courseweb) documenting that you attended your experiential site for the required number of hours.  The EValue Evaluations and the Hours Form are required elements for successful completion of your Community Pharmacist Practice 2 course.
