[Rx2021] Reminder: Graduation Application - Due 12/7/18!!

Hello Class of 2021,

Just a friendly reminder to PLEASE be sure to complete the application properly or the University Registrar’s Office will reject it when I forward your applications as a class.  Turn in your application to me in Student Services by Friday, December 7th.  If you prefer, you are welcome to scan and email or fax your application.



If you qualify for a minor, double major or certificate, turn in your application to me.  Do not turn it in to any other college or department on campus.



If you do not want to receive the BSPS degree, you need to email me in writing by the deadline.

Dear Class of 2021,

The Bachelor of Science in Pharmaceutical Science (BSPS) degree option is available to P2 students after successful completion of the Spring 2019 term.  The BSPS guidelines are attached and it is EXTREMELY IMPORTANT that you read the guidelines. If you are interested in receiving the BSPS, please submit the attached Graduation Application (pages 2 & 3) to me by Friday, December 7, 2018 in 229 Salk Hall.  When completing the graduation application, please remember to include IF you qualify for a minor, double major or certificate.  Please follow the directions on the application and do not use all capital letters when completing the application.

PLEASE double check your address in PeopleSoft to ensure that it is up-to-date even if you choose not to apply for the BSPS degree.

If you do not qualify or do not want to receive the BSPS degree, please email me in writing that you do not wish to receive the degree by Friday, December 7, 2018.
