[Rx2021] Flu Shot Reminder

Dear Students,
As a condition of participating in Experiential Learning activities, all students are required to receive an influenza immunization (flu shot) by October 1st.  Please document this on the Influenza Form - 2018, which can be found on the EValue home page and in Castle Branch.  The form must then be uploaded into your Castle Branch account.  Failure to obtain a flu shot by the deadline may preclude you from attending experiential activities at a number of sites.  Other sites may require you to wear a mask while on site if you haven't been immunized.  If your site does not allow you to attend a rotation because you haven't been immunized, the School may not be able to find an alternative arrangement, potentially affecting your ability to progress in the PharmD program.  Thank you for your attention to this requirement.
