Please only sign \nyour name in a blue highlighted space - the number of spaces listed are the \nmaximum number of students, please do not add additional spots. 3. Only \nchoose dates that you are able to commit to - the clinic is relying on your \nhelp for it day. If you need to change dates, please find a classmate to \nswitch with. You are not permitted to remove your name from a date without \nfinding another student to switch to your date. 4. Please add a phone \nnumber AND email address after your name. 5. If clinic is cancelled, this \nis how the clinic staff would get a hold of you. We try not to cancel, but \nsometimes emergencies happen..
