<b>2-Step </b>Tuberculin Skin Test<b> </b>completed and the results posted to CastleBranch by
<b>April 26, 2019</b>. The necessary form is attached to this</span><span style="font-family:"Times New Roman",serif"> e-mail. Your
<b>HIPAA Training module</b> must be retaken and posted as well. The HIPAA training site will not allow you to redo this until it has been one year since you last did it, but you must do it when you have passed the one year point. While you are looking in
your CastleBranch profile, please update any other items that are required. Your PA Pharmacy Intern License, CPR card, HIPAA, Bloodborne Pathogen and Infection Prevention trainings, and all immunizations should be complete and uploaded to CastleBranch. Failure
to have all of this taken care of by the due date will prevent you from starting your scheduled rotations and your summer underserved experiences (should you want to do the underserved experiences in the summer). If you have any questions regarding your CastleBranch
