<p class="MsoNormal">This is a reminder that you need to complete <b><u>six (6) hours</u></b> of patient care-related project time and turn in the required documentation before the end of this Spring 2019 semester. There are many students in each class who
have turned nothing in yet. Forms are in Courseweb in Community Pharmacist Practice (P2) and Health-System Pharmacy (P3) courses. If you have completed some or all of your hours, please turn in your documentation now rather than waiting until the end of the
semester. Please take care of this requirement before Friday, April 19<sup>th</sup>, the last day of classes for the Spring semester.<o:p></o:p></p>

<p class="MsoNormal">Please keep in mind that all projects require a pharmacist or other properly credentialed health professional to act as a preceptor for your project. Do not wait until the last minute to find a preceptor, even if your project was previously
approved. All projects AND preceptors must be pre-approved <b><u>at least one week</u></b> before the date of the project.

<p class="MsoNormal"><i>Failure to complete this requirement will result in a grade of incomplete for your Community Pharmacist Practice (P2) or Health-System Pharmacy (P3) course, stopping your progression in our program until the requirement is satisfactorily
completed. For P3 students, this means you will not be able to start your APPE rotations on time, resulting in loss of rotations.
